Price Chopper Selected as a National Partner in APhA Foundation’s Project IMPACT: Diabetes

Schenectady, NY - Price Chopper Supermarkets announced today that its pharmacy operation based in Schenectady, NY has been selected as one of 25 nationwide - the only one from the New York State - to participate in the APhA Foundation's Project IMPACT: Diabetes, a national multi-year initiative to improve care for people disproportionately
affected by diabetes across the United States.
Price Chopper Pharmacy is serving as the lead organization for the Schenectady County Community in Project IMPACT: Diabetes. Headquartered in Schenectady, New York, Price Chopper has developed comprehensive, patient-centered diabetes care programs, including its signature Diabetes AdvantEdge and HealthyU Diabetes Education programs, which have saved consumers more than $2 million since their inception in 2010. For those seeking nutritional guidance, Price Chopper also provides NuVal, the revolutionary point-of-purchase nutritional system that scores virtually all products in the store from 1 to 100 - the higher the score, the higher the nutrition. Through Project IMPACT: Diabetes, Price Chopper Pharmacy will partner with Capital District Physician's Health Plan (CDPHP) to expand these programs and improve access to care for patients who are disproportionately affected by diabetes. The project goal is to increase the percentage of patients who meet composite Healthcare Effectiveness Data and Information Set (HEDIS) measures and to improve patient outcomes in blood pressure, cholesterol and hemoglobin A1c through pharmacist-delivered education and training. The HEDIS measurement is a tool used by America's health plans to measure performance on important dimensions of care and service. Alisha I. Roberts, PharmD, AE-C, who oversees all of Price Chopper's Clinical Pharmacy programs, will serve as Community Champion.
